Job Description

Join ACE as a Community Intervention Worker and make a direct impact in the lives of individuals and families needing critical support. In this role, you’ll provide hands-on intervention services for participants through the Family Case Management (FCM) Model and Transitional Client Services (TCS), working closely with Intervention Care Managers to offer comprehensive support. Your responsibilities will include peacemaking, incident response, intake, case management, and connecting participants with essential resources in the community. As a liaison, you’ll foster collaboration and build bridges within the community, helping to create safer and more supportive environments for those in need.

ACE’s Community Violence Intervention (CVI) program serves ages 14 to adult, who are gang-affected or otherwise at risk of gang involvement. Through outreach activities and incident response, the program promotes community-level peacemaking and violence reduction. Through a phased program of participant engagement, potentially leading to cycles of family case management services, the program works to strengthen participants' family ties and other pro-social influences.

DUTIES & RESPONSIBILITIES:

  • Deploy to incidents as directed by the Program Manager to provide peacemaking, rumor control, and reducing tensions. Deployment is within the West Valley and can include hospitals, crime scenes, etc.
  • Reduce gang violence by reducing tensions, developing peace treaties, rumor control, community canvassing, conflict resolution, intervention services, and holding pro-social activities/events.
  • Serve as a point of contact in addressing community violence issues and provide the necessary resources to assist those involved.
  • Foster positive relationships with community members, partner organizations and local gangs and serve as neutral liaison between gang members and the community.
  • Work closely with the Program Manager to develop strategic plans to recruit potential Intervention participants, reduce violence within the community, and conduct events.
  • Perform outreach and marketing duties to recruit participants and educate the community on ACE services.
  • Participate in Intervention, cultural, and community-building events, including facilitating workshops at Summer Night Lights.
  • Carry out intake and eligibility determinations and make appropriate referrals when the applicant is not eligible for ACE services; conduct participant assessments leading to enrollment; complete and update participant service plans.
  • Arrange and participate in participants’ family meetings, service meetings, team meetings, etc. and coordinate wrap-around services with other providers.
  • Support all assigned clients by identifying key measurable short-term goals and objectives, using motivational interviewing, trauma-informed care, and harm-reduction techniques to help participants achieve their care plan goals.
  • Assist participants, parents, and families in accessing and navigating community-based social services and healthcare providers, including resources such as housing, healthcare, childcare, parenting support, Medi-Cal, CalFresh, and child welfare services.
  • Collaborate with the assigned Care Manager to ensure participants’ health and social service needs are met by monitoring care plan progress, tracking treatment and service compliance, adjusting interventions as needs evolve, and communicating findings with the Care Manager and interdisciplinary team, without assuming full care management duties.
  • Provide transportation of participants to meet program goals.
  • Enter data into participant databases, maintain accurate case files, and assist in generating reports as per ACE requirements.
  • Organize, coordinate, prepare, and deliver skills-based workshops for participants.
  • Facilitate workshops at Intervention events (e.g., Summer Night Lights).
  • Work to incorporate into organizational culture best practices for diversity, equity, and inclusion, restorative justice, and trauma-informed care.
  • Other duties as assigned.


KNOWLEDGE, SKILLS AND ABILITIES:

  • Sensitive to community, cultural and personal dignity of program participants and is able to successfully engage, assess, and plan services for participants typical of the program’s target population.
  • Recognizes appropriate boundaries and limitations on interactions with program participants and their families and the need to maintain professional standards and practices.
  • Familiar with standard approaches to accurate participant record keeping, including timely charting, notation formats, content limitations, and confidentiality practices.
  • Possesses knowledge of programs, regulations, and procedures regarding human services, social services, justice, and workforce development.
  • Knowledge of community/gang dynamics within the GRYD Zone.
  • Able to operate within ACE’s designated GRYD zone, exposing oneself to gangs found within the zone.
  • Able to work in assigned locations that may expose oneself to hazardous conditions or in extreme physical or discomfort or distress.
  • Ability to assess conditions and responsiveness to interventions.
  • Possesses necessary de-escalation skills to provide crisis and community violence intervention in the community.
  • General proficiency in navigating and using technology.


PHYSICAL REQUIREMENTS:

  • Prolonged periods standing and walking throughout facilities.
  • Prolonged periods of sitting at a desk and working on a computer.
  • Must be able to lift up to 15 pounds at times.
  • Must be able to bend, lift, stretch, climb, and crawl to provide services.


QUALIFICATIONS:

  • High School Diploma or equivalent community intervention experience.
  • Preferred but not required: Previous justice-involvement.
  • Formal gang intervention training (e.g. LAVITA) preferred.
  • Spanish language skills, verbal and written preferred.
  • Possess a current California Driver’s License, clean DMV record & adequate auto insurance.
  • Must submit to a Department of Justice Criminal background check.
  • Initial TB employment clearance required.

The anticipated pay range for candidates who will work in California is $28.85 to $30.45 hourly. The offered pay to a successful candidate will be dependent on several factors that may include but are not limited to years of experience within the job, years of experience within the required industry, education, etc.
